Defect Summary

Osram sylvania products, inc. (osram) is recalling certain sealed beam headlamps manufactured from february 3, 2014, through june 19, 2015, with part number h5006, that fail to comply with the requirements of federal motor vehicle safety standard no. 108, "lamps, reflective devices, and associated equipment." the affected headlamps may not meet the minimum intensity requirements.

Safety Consequence

These headlamps may not provide sufficient illumination of distant objects or the roadway. insufficient illumination could result in a vehicle crash.

Corrective Action

Owners will be instructed to return affected lamps to the place of purchase for a refund or replacement. the recall began on september 16, 2015. owners may contact osram sylvania customer service at hillsboro.cswebmail@sylvania.com 1-800-729-3777.
