Some universal joint yokes were forged with incorrect steel, resulting in brittle welds between the yoke and its mating shaft. Field service loads may cause failure of the weld and cause the yoke to separate from its shaft.
Corrective Action
The manufacturer will provide a replacement yoke or replacement yoke and shaft without charge to owner.
