Defect Summary
Wheels may have been fitted to the truck hubs without the required adaptor plates causing the wheel to remain loose on the hub. consequence of defect: a loose wheel could cause vibration and damage to the wheel and could result in the wheel separating from the vehicle which may lead to an accident.
Corrective Action
Install adaptors and properly torque wheel attachments. if necessary, replace wheel.
